I don't have a cusco one (I actually have a hybrid performance one, for sale on ebay), but it's hooked up to the smaller hose going into the intake. It's the lower one.

i have a cusco one. i mounted it on a freed up hole on the right side of the engine since i have a CAI.
i have it hooked up to the pcv valve on the left side of this pic.
