Causes of broken pressure plate...?

What are the causes of a broken pressure plate. I recently Ive broken 3 pressure plates, @ the rivets that hold the friction surface to the cover. Could engine vibration, or high rpm be causing this?
BTW : I am running an exedy organic disc.

Are They being properly installed and torqued in a criss-cross pattern?
