Some advice: Use the search before asking questions. Chances are your question has been asked before.

It wouldn't be too hard. You're gonna need a pulley to get off the front rotor piece thing that holds the lugs. I've done it before. The back ones though aren't as hard, as you won't need that pulley I've been told.
This is an involved procedure, use the search, if you get no answers to your question and are not mechanically inclined take it to someone to preform this conversion.
i just recently did this. i bought a JDM 5lug from a distributor which came with everthing i needed, including new 36mm axles. since the JDM ITR cam with bigger axles. if you get a CTR 5 lug or USDM 5 lug, then you wont need new axles. once you get the parts, it basically unbolting the parts that you need to and bolting in the 5lug stuff. it was a little time consuming. about an Hour a wheel. so it took about 4 hours for me. i also did a camber kit and comptech rear sway/tie combo at the same time. you will need an alignment after you do it, and you need new wheels and tire of course.
