Yes, about 8 inches down. Have it put in the top part of the header if its 2 piece.

the primary O2 should read all 4 cylinders. Placement is not really an issue, as o2 levels will not change as long as there is not an exhaust leak. The 2nd one, has no feedback to the ECU, its j ust for emmissions. You can usually just get an O2 simulator, or place it after the cat.
