The heavy duty ACT pressure plate is not designed to be used with a six puck disc. You will need to get an extreme plate to use that disc. The heavy duty plate is so chewed up because it could not hold against the shock of letting the clutch out and spun eating it up like that. Trying to use a stock one will only make the situation worse. Unless you have a turbo setup that warrants the use of a six puck, I would recommend having the pressure plate rebuilt through ACT and use that with a stock disc.

That P/plate has seen better days....go buy an extreme P/plate to use with that 6 puck disc, not the stock plate. It will grab hard as f&*k, believe me. Enjoy the harsh engagement and clutch chatter!! j/k Make sure you resurface your flywheel to lessen the chatter....but it still will a little due to the unsprung ACT disc.
