Balance belt not set?

Balance belt not set?
I had some difficulties getting my timing belt to tighten so I took my car to a backyard mechanic. He got it tightened but now my car sounds really rough at idle, sounds like something is loose inside the block. Could this be because he did not set the balancer belt at TDC with everything else?
