Adjusting a cam gear
Hold the 12mm bolt while loosening the screws holding the outer rim to the center, adjust the position, retighten the screws, let go of the 12mm bolt.
That 12mm bolt is going to want to move on you so hold it tight or note where it starts so you can position it accordingly.

Would you recommend holding the 12mm bolt with a wrench?
As for adjusting the cam gear, do I adjust it by moving the 12 mm bolt or by trying to move the belt?

The belt will pretty much stay put since its tied to the crank and its position.
