WTF!! I checked on my old 96 Accord that was totalled twice. It only showed that it was in an accident only once...the last time.
I ended up buying the car back and getting it fixed myself the first time. I still don't get it.

I think carfax is pointless.. you can't get ANY idea of what the car's been though with it. The only way it can have a complete history of vehicle accidents, is if the insurance company was involved EVERY time, and paid for the damage/repairs. Otherwise, if doesn't tell you ****. I personally know through several cars that it didn't have record of any of the accidents of these several cars... it's crap.
We use carfax at the dealership. But Preludestyl is right, if the cars damage was never reported to an insurance agency then it will not hit carfax. Also I think if it was reported to an insurance company, there has to be over $8000 worth of damage for it to get reported to carfax. Correct me if I'm wrong, I know you will.
However the good point of carfax is seeing where the car has been. I know I would not want to buy a car that has been in the Minnesota area due to the amount of salt used in the winter. It also lets you know how many times the car was reregistered, basically how many owners.

Every state has it's own laws and regulations on what must be reported. Some states require pretty much everything to be reported to the DOT or other body, some states require very little. That's what screws stuff like carfax up. Oh, not to mention a car totalled here in PA could be sold to someone in Texas or somewhere, rebuilt, and it wouldn't be required to have an R title (or atleast that's what it's called in PA, the "Reconstructed". Same title you're suppose to get if you swap the motor, which no one follows either)
