First post here. I could not find anything about this specific problem so I wanted to ask here!
I am helping my nephew out with clutch problems on his 2005 Accord. At first it sounded like a bad throw out bearing.
We tore it apart yesterday and when we pulled out the pressure plate, the clutch pilot bushing fell out! I was baffled that happened. As a result, the transmission input shaft bearing/seal is fried.

The only two explanations I can think of are:
1. The bushing was installed incorrectly. I unfortunately have no context for that as this is a used car and the bushing was completely out of the bore upon disassembly.
2. The OD of the bore in the crankshaft is too large for a proper press fit on the split bushing.
I am going to take some measurements of that bore and see if I can find any tolerances on that.
My biggest fear is if I go and press in a new bushing the same thing could happen again. The uncertainty is killing me. It would be awful if this happened again and killed another input shaft bearing after all of this work.
Anybody happen to experience this same scenario?
Any help or guidance would be greatly appreciated!
