Default Replaced Timing Chain and P0341 came back 4 cylinder

Hi All,

Looking for some help. I had error code P0341 Camshaft Position Sensor A.
I replaced the timing chain and was good for about 3 months-3000 miles and now the error code returned.
I pulled the valve cover and went to TDC on the crank-white mark.
Looks like the VTC is advanced a tooth or 2 (4 o'clock vs. 3 o'clock) as well as the fixed gear (10 o'clock vs. 9 o'clock).
I tried to line them up but could not and plan to tear down.
Any ideas what happened? it is a fair amount of work to do this job and I would like to prevent reoccurrences.
Also, when I redo the work

, how to I "Re-Lock" the VTC like it was when new?
Thank you.
