Just picked up a 2013 EX-L 2.4 with 198k miles and old owner never serviced trans. Has anyone changed oil with this many miles or should I just leave it alone.

 

I'd take to a dealer for the regular major maintenance ASAP. Every 60k is when it's due. They always change the CVT transmission fluid. I have nearly 198k now and have done it twice. The 9th Gen Accord is a tank if you maintain it, my 13 Sport auto has only had one repair (the starter) in 194k miles so far.
