Default 4-5 lug conversion for a 96 Honda civic ex sedan

I was wondering how to do a 5 lug conversion for my 1996 civic ex sedan, how would I go about that?

Easy button (installation wise) - find a 96-00 Civic Type R five lug conversion from an importer (HMotorsOnline, etc), swap everything over.

This way gets you fresh hubs/bearings/etc:

Front - Press in USDM Integra Type R hubs with new bearings, install ITR rotors, calipers and pads.
Rear - acquire 4x100 rear spindles or trailing arm assemblies, Type R calipers, pads, rotors, hubs. Use this kit from Honed: 5 Stud Rear Hub Nut Kit - Honed Developments

Use E-brake cables for 99-00 Si. Use Integra 1" master cylinder.
