I have a 2000 Civic ex that I swapped from auto to manual.
Car is stock, for the most part other than the swap.
Running the manual 5 speed intake with proper IACV. Running the ECU that's supposed to be for Manual with VTEC. (37820-P2P-A12)
I looked through different guides and was able to get everything working for a bit before this issue came up. After running the car for a bit (15-20min) revs will start to hang, sometimes around 1.5k or 2k. Sometimes turning it off and on will fix it for a while before it begins doing it again. I already swapped and calibrated the TPS sensor since I thought that was the issue at first. Here is a video of what the car does. https://youtu.be/Vsdro4YbUb8A couple of days ago CEL came on reads Idle Air Control System. Not sure what to tackle next. Any advice is greatly appreciated.

The instructions that you posted above don't even make sense. Here is what MUST be done. You will connect the Yellow/Black (or Black/Yellow) wire from the original 3-wire plug to one of the two wires on the new 2-wire IACV round plug. The other wire on the 2-wire plug will be connected to the Blue/Black (or Black/Blue) wire from the original 3-wire IACV connector. The Orange/Black wire will not be used.
Now, here is where things get a bit complicated. The ECU plug end of the Blue/Black wire must be removed from it's current pin location and installed into another location. The Orange/Black wire will NOT be used... or even connected at either end. Some conversion/adapter harnesses have this Blue/Black wire modification already "built in", but if not, you will need to move the Blue/Black wire to it's proper location. I always suggest properly moving the Blue/Black wire to the proper location so even bad/cheap adapter harnesses don't have IACV issues. The Blue/Black wire will be found at position B6 (top row, 3rd position from the right end) and needs to be moved to B23 (bottom row, 3rd position from the right end). These descriptions are from the wire side of the ECU plugs.
Sorry for the delay. It seems my cables were flipped, and the valve itself was also damaged. Got a used valve from the junkyard that looked in really good condition. Car doesn't limit the revs anymore and CEL is gone. Thanks for your help.
