Engine noise 2008 Honda Civic
My brother thinks it's broken VTEC. Could also be timing chain. Anyone know what it is?

Oil and coolant levels okay?
Is the Check Engine Light on? Any trouble codes?
If fluid levels okay, no Check Engine Light and no trouble codes, it may be the due to a failing water pump. If it is the water pump, eventually, the water pump may seize.

Belt tensioner, take a closer look at that.

Bolt attaching belt tensioner to engine broke, thus, hitting the water pump pulley.
