Default 03 civic manual clutch release fork bent

The car has been driving totally fine for many years, then a few days ago
I had a broken clutch master cylinder and the clutch pedal would stick to the floor. After sending it to the local Honda dealer for replacing the master cylinder, they suggested replacing the slave cylinder as well. I said ok. One day later they called me saying the clutch release fork is broken and the whole clutch needs replaced. Here is picture of the release fork, i asked some mechanic friends around they said they never saw a clutch fork broken before, the way it looks something inside must be broken. Also I noticed some metal marks under the fork on the hole where is sticks out (marked with a circle). can someone tell me how this clutch could have been broken? Thanks!
