1991 Honda Accord Ghost Revving
New IAC, new hoses, no air leaks that I can tell. "Burped" the damn thing excessively. It still ghost revs... I'm using a no spill funnel. It also "Surges" when trying to burp it after about 10 min.

If that is the case, your IACV seems like the cause of the super fast idling. Cleaning the IACV might work, but if it is an original part on a '91, it might have reached the end of its life.
Update: You said the IACV is "new," but you should still check it. Also check the FITV, Fast Idle Thermal valve. It slows the idle as the engine warms.

You bleed the system with the engine off/cold.
Car should be parked on a flat level surface.
Open bleeder screw, for less mess attach a clear vinyl tube(fish store if not hardware store) to the bleeder tip.
Watch for no bubbles, a long enough hose the coolant will just level out and you can watch the coolant for air bubbles,
Open radiator cap, verify the rad is full.
Gently squeeze the upper hose and note if any air comes out of the bleeder nipple.
If no air or bubbles, close bleeder screw. remove tube.
Verify radiator is full.
Verify coolant reservoir tank is properly filled.
Close radiator cap and radiator reservoir.
Start engine and allow to warm up.
Watch temp gauge, it should settle slightly below the mid way point on the gauge.
Look for any leaks.
Close the hood and wait for the cooling fans to come on(idling) while watching the temp gauge.
If all seems well go for a test drive.
With the engine fully warmed up.
Remove the air intake hose to the TB.
Start engine.
Place your finger over the IACV intake port inside the TB.
This will block off the IACV completely. If the IACV is not returning to it's full rest position it may cause a false base idle speed.
If the engine rpm drops but runs rough then there is something else going on with the engine.
Engine should run smoothly.
Verify the TB throat and butterfly valve are clean.
Verify the ignition system is in proper order.
Verify the valves are properly adjusted.
