Blown head gasket?
A badly blown head gasket would leak ... coolant into cylinders, gas out during compression stroke
Not so bad, there might be rough idle, exhaust gas in the coolant, might see some creamy stuff in the radiator
Hard to say if it is or isn't. It can be tested by a kit that checks for exhaust gases in the radiator

It sounds like you are seeing oil smoke from worn rings. This kind of smoke gets worse when you floor it and continues until you let off the gas.
The first symptom of a leaky head gasket is losing coolant from the radiator, leading to overheating. Only in a severe case will you see water "smoke" from the tailpipe. This "smoke" is of course actually steam and it will smell like antifreeze.
