Default 98 HONDA TRANSMISSION PROBLEMS

I got a 98 honda accord, with a 4cylinder in it, automatic transmission. Yesterday, when I put it in drive, it would take a couple of seconds to go into drive and it would do it with a bump. While driving when it shift gears it seems like it comes out of gear and the rpms run up really high. If I take my foot off of the accelerator then it would slip back in to the gear. Does anybody habe any idea of what this may be?

Verify your transmission fluid is full.
Verify there are no stored codes, use an OBDII code reader/scanner. If any, post them back here.
