Shift solenoid failure

I have a 93 Honda accord lx automatic. I have replaced both shift solenoids twice and I have replaced my tcm but I keep getting a shift solenoid failure an was wondering what else could cause this?

What reason do you think it's the solenoids?
What codes(D4 light) were you getting?
