If the D4 or S light are where you got the code Code 5 is for the shift console switch. Either a short in the wire or the console switch. Verify it is properly adjusted and at this age it is probably dirty.
Remove it, clean it, reinstall and adjust it.
If the CEL lamp is where the Code 5 is from, your MAP sensor, or wiring, may be damaged. Verify it has correct linear output by backprobing the sensor with a volt meter.
