Default 94 Accord F22b1 / Fan on w. car cold + rough idle

Cant seem to figure this one out...

My 94 Accord EX (F22b1 VTEC Motor), car is cold, so when I put in the key immediate the fan turns on and car starts off with a low idle then jumps and holds at 1500 RPM, sputters before it gets to operating temp

From search around with guides from motors similar as I cant find guides dedicated to the F22b1, I have done:

Vacuum test (no leaks found anywhere)
Cleaned both IACV and FITV,
Adjusted FITV Screw, throttle cable
Cleaned throttle body
New Plugs, wires, dis, cap and rotor, cleaned K&N Drop in filter
Checked O2, unplugged it and car still was at high idle
Coolant is topped off
Leak Down test shows consistent 190 ish

When these were all done at once, I unplugged the neg of the battery and removed the ECU backup fuse.

No CEL on the car I might add. but still no change...

any insight?

O2 won't affect idle speed. Something is causing the fans to come on and at the same time cause the IACV to dump alot of extra air in the engine right at startup.

My inclination is to replace the coolant temp sensor. If it is malfunctioning it could be telling the engine that it's 0* kelvin, or a more reasonably-silly cold temperature richening the fuel maps and opening the IACV to full duty cycle. However...it calms down when it warms up?

**UPDATE**

Well coolant temp sensor, checks out to be at spec...car start on the first try and never struggles to start.

Went for a drive and now the RPM drops down to around 600 RPM, when at idle and at operating temp

and then jumps up to 15000 RPM when I put it into Gear, then RPM drops down to 600 when at idle...i can also smell faintly coolant and fuel

So I pulled the plugs after this drive and noticed they are black, which indicates that it is running rich...and the coolant is all topped off...

Any other ideas?

I'm not sure what the issue is either. The 02 sensors don't do anything until the car is warmed up. Is there any difference with the rough idle when the engine is warm or cold?

Here is a new update...

I boggled the idea of the ECT (Coolant Temp Sensor) and decided to change it, as well cleaned and updated OEM ground on the intake manifold plus added a second ground from the intake manifold to the chassis using 4 gauge wire.

Then had the Radiator flushed from both the radiator itself and a bolt near the oil filter, followed the procedure to bleed system, while under there as well noticed the pipe that holds the 02 sensor, was cracked on top, so that was replaced as well with a new 02 sensor (despite the 02 that was previously on there was changed 6 months ago but fused on)

as well using Mopar combustion chamber cleaner through the Brake Booster hose and TB, changed the fuel filter and found a OEM fuel pressure regulator from a H22 at the mechanic shop (FPR was free, it was OEM, so why not)

All of these..

seems to have stabilized the idle, despite being on the high side at around 900 RPM and an occasional sputter.

"15000 RPM"? I don't think that's possible.
