As of now - 27 Nov 13 5:38pm CT - I have located and paid a board member for this module.
Thanks for everyone's help.....and for just for info part is needed to pass my safety inspection on my daily driver.

I'm looking for a Belt Warning Control Module for my 1990 Honda Civic Lx
The original module is a RK-0221. The data on top of the module reads:
RK-0221
SH4 12V
Lot No: 0424U
CME
This module controls the automatic movement of the over-the-shoulder seat belt located in the frame above the door window.
I have tried a module with a part number: RK-0091. This module corrected the fault on the driver side, the passenger side did not respond to this module.
I have done all the testing of the relays and motors thus concluding this to be the faulty part.
If you have a 1990 Civic LX in your salvage yard, you will find the Belt Warning Control Module behind the stereo/ashtray on the floor.
PM if you can help me with this.
Thanks

The module is located on the floor behind the Front Console Panel.
The Front Console Panel is the panel below the center of the dashboard that covers/frames out the climate control *****, radio, ash tray, and cigarette lighter.
There are 4 phillip head screws holding this panel on; 2 on the right side top and bottom and 2 on the left side, the one on top is inside the corner of the glove compartment (open the glove compartment and it's on the left side of the door) and then 1 screw is on the bottom of the panel.
After the Front Console Panel is off you'll find the Belt Warning Control Module right behind the frame that the panel was attached to. The module has a black cover on it. On the right side (passenger side) there is 1 phillip screw that needs to be removed. This screw holds on the access plate of the black cover; once this access plate is removed you then have access to the two multi-pin connectors attached to the Belt Warning Control Module. Remove the two electrical plugs.
There is no need to remove the rest of the black cover.
The Belt Warning Control Module is secured to the floor by two 10mm nuts, one on the right side and one on the left side. This is a tight spot on the left so you may need a 1/4 drive ratchet or a ratcheting wrench. This nut only needs to be loosen, not completely removed. The nut on the right side needs to be removed, after moving this nut you can slide the module out from the right.
You can now remove the black cover, on the top of the module you'll find the part number.
Thanks for the effort, I appreciate your time.

There's 10 or 12 of these modules (with different Part Numbers) on eBay Motors going for anywhere between 12 and 45 dollars with shipping between 5 and 15 dollars. A few of them are Free Shipping. Most of the modules are in the 20 buck range.
I'd pay 40 buck with 10 dollars shipping if it's a RK-0221 module.
Is that good for you?

Here's a photo so everyone knows what this looks like:

The P/N on this baby is RK-0091.....I'm looking for an RK-0221 or a verified module that came out of a 90 Civic LX. I'm not sure if the 89 and 91 models used this RK-0221 P/N.
As far as a Honda part number, I believe there were two different numbers used by the dealers; one superseded the other. I don't know what those numbers were.
That's it.....whatcha got?
