Brand New Clutch: Slipping? Glazed? Hot Spots?

Hey everyone.
I had an auto to manual transmission conversion completed just over a month ago. I had an S40 B000 transmission with Quaife LSD, an Exedy Pro Kit clutch and Competition Clutch 11.4lbs flywheel installed. I gave the clutch a generous break-in period with painful self control. Just to clarify, everything but the transmission is brand new.
In the last couple of weeks I've noticed that after some time driving (i.e. rush-hour traffic, frequently up/down shifting), the clutch almost feels like it's slipping when up-shifting. For example, if I shift from 2nd to 3rd, or 3rd to 4th, as soon as I disengage the clutch the RPM's go up about 500-1000 revs before dropping down to match the engine. After driving for a little while free of frequent shifting or letting the car cool down, the problem seems to go away. I also tested what would happen with the revs at 1500 in 4th or 5th and stomped the gas to see if it would slip but it wouldn't. But when it was at around 2000RPM in 3rd and I did the same, the revs moved up slightly before normalizing.
I'm trying to figure out what the cause is. I don't think the clutch is fried. It doesn't seem like it and I don't notice that distinct smell.
Could it be glazed? From what I've been reading, I don't think it is.
I was also wondering if it's possible that the clutch isn't fully disengaging and this is causing it to get hot and creating hot spots until it cools down. I read this happened to someone on another forum.
Any ideas?
