My honda jerks when taking off and it does the same on incline or doing down hill anyone have any suggestions as to what this might be

 

Does your trans. shift funny when this happens such as jumping RPMs? Do you hear a clunk because that could mean engine mounts. Try checking them for cracking. Another test for mounts is visible vibrating of the steering wheel during idle. What's the mileage on the car?
