With all 3 being high my first instinct is it's running rich due to a bad O2 sensor or a bad fuel pressure regulator but, with a high mileage vehicle it could be any number of things. It may be you just need a tune-up. I would start by seeing if it has any codes and going from there.

How to check for codes: http://check-engine-light-codes.blog...retrieval.html

Seems like a misfire would cause the emissions failure, I would suspect the car does not run smoothly either. Fix the misfire the car will run much better. Fix it quickly as a misfire like that can permanently damage the catalyst.

When was the last time the car was given a tuneup and systems check?

Check the exhaust manifold. It might have a crack in it.
Have you done a compression test?
New plug wires are not always in working order.
Check fuel pressure is within spec.

There is something goofy going on at idle, you have nearly 4%O2 along with nearly 1% CO. This only happens if there is a misfire.

CO and O2 are inverse of each other. Both will not be high unless no combustion is taking place.

Remove/replace a plug wire at a time until the engine is not affect by the removed wire. That then is the most likely bad cylinder.

Do you mean, how many catalysts does the '95 I4 EX have?
One.
Exhaust manifold > down pipe> O2 sensor pipe > catalyst.

But don't even think of spending money on a new catalyst if you have yet to fix the previous issue. The bad running will kill the new catalyst.

Not an expert on smog tests, but can tell from experience.

I second MAD_Mike, also 1994/95 not necessarily through CEL if there is a misfire, check by replacing each wire, if there is any stall in the engine.

Did you replace Air Filter? Cleaned EGR Ports? What kind of Spark plugs are you running? What's the condition of the Dist cap?
