Default Transmission grinds

I have a stock 97civic with a stock d16 and third and fifth gear grind once I rev past 4000 rpm.

I changed my transmission fluid but that did not work. So what can be the cause of the grinding?

Worn 5th gear sleeve most likely, but you wont know for sure until you take it apart. Could be a worn sleeve, hub, gear or some some combination of all 3, along with a worn 5th gear synchro.

Could the fact that it is leaking fluid also cause the grinding?

3rd needs a sleeve assembly (includes the MS 3rd for the D series), 5th needs the same more than likely.

It's not so much that it's a hard job, but the parts cost more than the average used D-series transmission. That said there's no guarantee that a used transmission wont have the same or some other problem(s)

A 2nd or 3rd gear grind is common on most used transmissions now. 5th grind is a bit less common.

What causes the grinds? Or is it just common for all cars as they age?

Rounded or flat sliders and gear dog teeth, worn shift forks, worn synchros. Those are the top reasons.
