Default Element clutch or gearbox problem?

Hi Guys,
I have an 03, 2 wheel drive, manual with 236,000 miles on her.
About 3000 miles ago I was driving slowly around town when the whole cast suddenly shunted forward as I changed gear. I quickly dipped the clutch & coasted to a stop. But then I couldn't get it in any gear at all.
I sat waiting for the roadside rescue, but when they arrived it drove fine & I've driven another 3000 miles without any issue. Until two days ago...
I had to drive through a very large puddle which turned out to be deeper than I thought. I got through OK but now when the car's under load pulling out of a hilly junction, say, it shunts forwards again & then it's fine.
It also did a little jump forward when I started it the next morning (when it was in gear) so I'm assuming this is a clutch issue.
Any thoughts..?
Many thanks in advance.
