What are the causes of BOTH the low beams) to not work at the same time?

Check to see if both bulbs are good. You'd be surprised how often both go out. If they're both good, check for power to the bulbs. If no power, see if the relay is bad. If the relay is good, check for power to the relay

Summary:
ON CERTAIN PASSENGER VEHICLES, THE LOW-BEAM TERMINAL ON THE HEAD LIGHT WIRE HARNESS CAN OVERHEAT AND COULD CAUSE THE LOW BEAMS TO FAIL WITHOUT WARNING.
Consequence:
AN UNEXPECTED LOSS OF LOW BEAMS COULD RESULT IN A CRASH.
Remedy:
DEALERS WILL INSPECT THE HEAD LIGHT SWITCH AND COUPLER FOR SIGNS OF HEAT DAMAGE. IF HEAT DAMAGE IS PRESENT, THE DEALER WILL REPLACE THE SWITCH AND COUPLER. IF NO HEAT DAMAGE IS PRESENT, THE DEALER WILL REPLACE THE HEAD LIGHT SWITCH AND ONE MATING PIN IN THE COUPLER. OWNER NOTIFICATION BEGAN ON APRIL 5, 2004. OWNERS SHOULD CONTACT HONDA AT 1-800-999-1009.

If you think this covers your car, take your car to a Honda dealer. If your car is covered under the recall, Honda will replace your headlight switch at no charge.
