Default Hydro B-Series Clutch, Pressure Plate or Flywheel Issue

The car is a 95 Civic Ex with a B20B B16 trans. Exedy lightweight flywheel with Ebay 6 puck clutch

The problem started while driving, the clutch pedal only held pressure about an inch above the floor. I replaced my slave cylinder, but while putting the new one on I noticed my clutch fork wouldn't disengage all the way.

When I pulled my transmission off I noticed the teeth on pressure plate (not sure proper name) were pushed in, instead of bowing outward.

I purchased a slightly used ACT clutch with pressure plate and proceeded to install it but noticed as soon as I started to torque the pressure plate down they would start to bow inward.

My first thought was flywheel so I purchased a stock ITR flywheel and it is doing the same thing with both clutches. There is more room between the pilot tool and the pressure plate teeth on a certain side.

If anyone has an ideas or input please help me out!

are the disk and the PP mismatched parts? if the disk is to thick that will happen.
