98 Honda Accord V4, Shuts off while driving

My 98 Honda accord has recently started having issues (started maybe a month ago). While driving at low RPM (usually doing 3-25 mph) the car has shut off while driving. Once it shuts down, it will keep rolling until it stops. I shut the ignition off and it re-starts back up with no noticable issue.
Sometimes it seems like this will occur, but if you give it some gas it seems it prevents it from shutting down, however the "Maintenance required" light will turn on for ~5 seconds and the car beeps about 5 times. You can tell it is about to occur because you seem to lose throttle in the moments leading up to it shutting down.
The number of times it has actually turned off while driving down the road is only two, however I have had at least 10 instances where it nearly shuts off and I get the dinging and maintenance light flashing.
I have had my check engine light on for awhile. I had some exhaust problems a year ago, and It never went off after getting the catalytic converter and exhaust piping replaced. I got the OBD2 scan yesterday, and the two error codes still relate to the CC, P1456 and P0420. I looked these up and don't see anywhere that they would cause the car to shut down while driving, but I am not a mechanic by any means and may have misinterpreted some information.
I believe the P1456 error code has caused my check engine light to be on for most of the time I have owned this car, the first time it came on I took into the dealership and they stated it was likely from not tightening down the gas cap enough. Ever since I have been hesitant to over-react to my check engine light, but this might not be a good way to handle it.
If anyone has any suggestions on what may be causing this, or any information on similar problems, your adivce will come greatly appreciated. Thanks for taking the time and have a great holiday weekend all.

Sorry, the vast majority of us will not be able to help as I seriously doubt any of us have ever seen, much less worked on, a V4 Honda engine. Are you sure you don't have an I4 engine?
you should have the EVAP system inspected.
I'd reset the codes and hope that they don't come back.
the stalling issue may be related to the ignition switch recall.

I will try to set up something this evening to have the EVAP system inspection done.

Update - BP announced they had a large batch of gasoline get to the conusmer in the Chicago/Milwaukee area, and the problems I have been experiencing seem quite similar to those they describe (Rough startup, car stalling, etc).
