I had a similar problem and mine I believe came down to my Bisimoto/Evolution Ind. "8.9" lb. alum. flywheel. Ran this FW brand new out of the box paired w/ an Action 1MS clutch. 17K miles later, BOOM, springs and metal shards shot out of the disk poking a hole in my trans. 2nd clutch, CC Stg 3 Ironman, lasted 7K miles, same thing, springs rattled to death. 3rd clutch, CC Stg 4 warranty replacement, lasted 5K miles, again, same thing. Not a big deal, but the FW actually weighed 9 lb 13 oz, go figure.
Final got so fed up w/ the situation and my DD being down so many times for work, I bought an H22 clutch and brand new Honda FW from the dealer, best decision I ever made @ the time.
In between clutches I did not have the FW resurfaced, I'm not sure if this was a bad idea or not but visually and w/ a long straight-edge, it checked out fine. There was something about that flywheel though, I have feeling it warped. I put it aside and haven't touched it since. I'm not the world's best driver either, but I know what I'm doing and what's good and bad for a clutch/flywheel.
Here's a ?, did you tighten your pressure plate bolts in a criss cross manner or did you work your way around? I did mine working my way around in the smallest stages b/c I felt criss cross would've taxed the alum. threads too much since the pressure plates were pretty stiff. Maybe this was my mistake, IDK.

You're supposed to do a criss-cross or alternating pattern to tighten anything, it prevents the metal from warping while cool and worsening further still when it gets hot.
Are you putting this "stage 4" clutch on an otherwise fairly stock engine? Someone on another site compared that to, I believe, using a magnum-sized condom when you're not... uh, equipped for it. Pretty pointless.

Stage 4 on an H22 swap, is it boosted by chance?
How are they failing?
Have you contacted Competition to see what they think? 4 complete clutch kits in 2 months = something f#@ked up!
Was it a rigid or sprung kit?
Did you use the corresponding Competition pressure plate each time?
What method are you using to break them in?
