Default B18A1 valve clearances & other Q's

I recently swapped a '91 B18A1 converted to OBD1 into my Del Sol, and I have a few things I still need to fix.

The valvetrain is tapping pretty loud, and I plan to adjust it to quiet things down a bit. I got a set of B18B cams off a friend, so if I drop the B cams in, should I use the B18B clearances? Are the heads identical in these motors, cams aside?

When I dropped the motor in, I used a B18B intake mani, and the throttle body from an OBD1 B16A. I've had cold start issues since I put it in. It idles really low when I first start it, and I have to keep my foot on the pedal or it dies on a cold day. There are two idle air controls on it (I believe), which one do I need to fix/replace to resolve this? The one on the intake or the one on the throttle body?

Lastly, my radiator fan hasn't been coming on, so I bypassed the relay. However, I don't want to leave it like this. The relay seems to be fine (swapped it with one I know that works). I've read there's a thermostat switch or sensor that controls when the fan comes on, maybe mine is dead? Where is it and how do I check to see if it quit on me?

Could the radiator fan problem be related to the coolant temperature sensor? My cluster reads fine though.
