Default code 14 issue (iacv)

Helping a friend with his 93 Honda Accord to fix a code 14 and trying to get it to pass CA IM240, replaced the exhaust manifold due to a crack, new bosch O2 sensor, ECU, and new denso IACV. Installed everything properly, and still is showing a check engine light, same code. We're at a point where we arent sure if we're missing something, any suggestions?

my 94 civic with a b16 did the same thing and i replaced the sensor also and didnt do anything my problem was the fast idle thermal valve which is located on the bottom of the throttle body and is where one of the water lines that goes to the ivac comes from

i took it apart and cleaned it and screwed the little plastic thing back in and fixed it but after that i just bypassed it
