Default Clutch chattering at higher rpms

Hey, I have a 1991 honda civic Dx with a 5spd..I got the clutch replaced at a local mechanic.. and it seems like he messed it up. It has been like this for about 20k miles and it has not gone away. It usually does it on uphills because you need more gas to do it, although it seems to be getting worse and I can feel a bit of a vibration at normal take off. If I do clutch and gas at the same time it doesn't chatter..The chatter isn't a little chatter, its shakes the entire dashboard at high rpms...

I hope you guys can help me out, and point me in the right direction of either a new clutch that I will do myself, or transmission or other mounts.

thats what it says on the receipt. Although there seems to be an odd separation on the transmission mount. like they forgot a bolt or something. I will have pics of it sometime today.

either they didnt machine the flywheel smooth, adjust the clutch cable, or like you just noticed, forgot a bolt to the trans somewheres.

I have a picture, I just have to upload it. The transmission mount looks odd. Should do it now.
