Code 16, Makes no sense! Help please!

bone stock swap, virgin harness and everything.
I was driving home after work, and all of the sudden my car started cutting out, and running in limp mode. When I got home, I checked the c.e.l. and its code 16, Fuel Injector. I replaced all four injectors with ones that came out of another running ls. I start it up, code is gone, car runs perfect, drive it about 10 minutes to the gas station, and as soon as I pull up to the pump, c.e.l. comes on, code 16, car is back in limp mode. Made it home, changed all four injectors again, along with f.p.r. and fuel rail. Still code 16, cars running in limp mode. WHAT THE HEEELLLLL!?!

Heres from the factory service manual:
1. Do the ECU reset procedure. If code remains go to step 2.
2. Measure the resistance between two pins on each injector. Is it 10-13 ohms? If yes go to step 3. If no replace injector.
3. Turn ignition on. Measure voltage between both injector plugs on each injector plug. Does each have battery voltage? If yes go to step 4. If no check continuity between yellow/black wires and main relay.
4. Reconnect injector plugs and turn ignition off. Measure voltage on ECU plug harness A23(-) and the following for 12V: Injector 1 (A1 terminal), Injector 2 (A3 terminal), Injector 3 (A5 terminal), Injector 4 (A2 terminal). Is there voltage on each, if not repair the open wire between each ECU terminal and the corresponding injector. If yes than substitute a known working ECU.
