you should mount one wherever the header collects so the o2 sensor is sensing ALL cylinders not just one or two.

on my stock jdm b16 manifold there is one bung in the number 2 flange and one in the number 3 flange, right where it goes from 4-2 about 5 inches from the exhaust ports on the head.

Correct, now if you pay close attention each O2 sensor senses for a pair of cylinders only. Installing one near the collector will only cause you fuel problems which can lead to hesistation during acceleration. Your primary O2 sensor on your stock manifold is nearest the driver-side (which monitors cylinder 1 & 4), and your secondary, which is closer to the passenger-side monitors cylinders 2 & 3. Plug the O2 bung near the collector and weld a bung on one of each pairs of cylinders. I suggest cylinder 4 and 2 (if I'm not mistaken they look to be the easiest with enough clearance). NEVER, and I say again NEVER combine both O2 sensor to one (unless you get your ECU chipped to run only 1 O2 sensor), other wise you'll run in fuel problems like I said earlier, and experience hesitation in the mid-RPM band range.
