Transmission popping out of gear
Ok so my friend and I were driving home tonight in my 1996 Honda Accord LX automatic trans and as we were going the car started jerking just a little. I barely noticed it, but he noticed it and said "is that your trans???" I thought he was just being paranoid so I kept going. Well about 5 seconds later I'm going the same speed and the rpm's go up without me doing anything different, almost like the car popped out of gear. So I pulled over and change through all the gears and the car won't go anywhere. We pop the hood and look and notice the dipstick for the transmission is blown out of the orifice. He finds the dipstick and sticks it back in and we crank it and the car goes back into gear so we start driving again. A few minutes later the car pops out of gear again, the rpms go up and we pull over. We turn the car off and crank it again and it goes in gear and we drive. We repeat this process for about 30 minutes until we make it home. It seemed like the car never would make it out of second gear and after about 20 minutes of driving the check engine light came on.
Does anyone have any earthly idea what would cause this or what in the world is wrong with the car?

I limped the car to advance auto parts and had them run the code.
It came up with
