I have the 1999 Accord.

The D4 on the gear indicator next to the fuel gauge is not lighted when
the gear selector is in D4
All the others (P, R, N, D3, 2, 1) are lighted fine.

I removed the instrument cluster from the dash and looked.
Unfortunately, I don't see any bulb behind the instrument cluster for the D4.
My guess is the D4 bulb is inside the instrument cluster.

Has anyone had experience with this problem and how did you fix it ?

take it to a tranny shop and get the solenoids checked
i had similiar problem on my 95 accord except the d4 light would flash all the time and it would shift weird then the light just stopped working
i took it in to a tranny shop and it was shift solenoid b that went out
they told me i was lookin at almost 250 bucks just for a replacement plus labor
went to the junkyard and pulled one for free, threw it on and solved the problem.

My D4 light doesn't flash and the tranny seems to shift normally.
The D4 light bulb simply doesn't light up anymore.
It may be a burned light bulb but the bulb is inside the instrument panel (IP)
which requires me to disassemble the IP.
If someone had opened up the IP, please give a how-to.
