Default Idler pulley fell off! Help needed!

So we just changed a serpentine belt in my friends RSX-S and right after start an idler pulley fell off. Spent another couple hours trying to put it back on and after trial start the thing fell off again like butter.
The question is - what the f*** is wrong with it? How should the pulley stay in without any bolt or nut?
Had to leave the car on a Shacks parking overnight which sucks
Help, please

never knew the dc5-s had a idler pulley, the ep3 has a tensionor pulley but you do not remove it to replace the belt, you just pull aganst the spring to unload the belt....Have you tried loctight?

thats your water pump pulley.....thats a press fit install...You could pull the waterpump and drill and tap the shaft and stick a bolt in it but I think it is time for a new waterpump....here is a link to a blow up of the whole mess....
