Transmission HELP!

Transmission HELP!
Okay heres the deal. When my motor is cold the van drives fine. When the motor and tranny warms up the Transmission starts to slip. R or D doesn't go anywhere. I got the car home now and its been resting for 4 hours. Now the Van drives fine.
What could be wrong? Torque Converter?
2001 Honda Odyssey 160,XXX miles. Honda replaced the Transmission at 89,XXX and now the tranny went out. What should I do?
