Default Code 15 after wiring vtec

Okay so I got done swapping the gf's hatch got it running without vtec wired up (94 dx) ran good for a day then last night I pulled it in my garage to wire up vtec. I got all finished and went to start it and nothing. got a code 15. I looked over my wiring to see if i possibly messed anything up but everything looks good. Do you think its possible for the ignitor or dizzy to just go bad like that without any warning signs?

Do you measure battery voltage at the Blk/Yel wire attached to the coil and igniter unit in the distributor?

Do you measure ~10V at ECU connector terminals A21 and A22 for the Yel/Grn wire attached to the igniter unit?
