Jolting Transmission

I have a b16 tranny that I did a rebuild on and can't pinpoint a problem but something occurred today but I am not sure if it is relevant to the whole dilemma.
When I take my semi built LS VTEC for drives, it surges just a little bit while driving. The RPM stays at the same speed but you can still notice it jolting then letting off and jolting. When I take my foot off the gas and let the rpm go down with the tranny, there is no surging.
Also, I just noticed that when I put it into 4th gear today the rpms were going up but the car was going no where. I pushed the clutch in and tried to put it in 4th again, only hearing a slight grind and then still no movement. On the third try I pulled harder and got it to work and it also worked for the 10 more minutes I drove.
It seems like a clutch problem but wouldn't the rpm's be altered. Is it something with the axles. I'm scheduling a tune for this soon so I need to find out what it is. Thanks for any help

el problimo is fixed. So glad it isn't the tranny. I made this in the tranny section since the 4th gear but still gotta figure that out. I had multiple threads since none got replies on this topic. I went to go re gap my plugs and check out what I found

I saw that I need to clip and clean my nails and this plug had no gap at all. Not sure how this happened but all is well now.
