Default h23vtec timing questions

Ok a question for the h23vtec or manual tensioner conversion guys. I have had a manual tensioner on my car for 2yrs recently i noticed that the motor jumps timing so how. The belt is tight as i can get it without making the belt make the whinning noise. I do have an older belt i will be getting one pretty soon. But i have redone the timing aleast 3-4 times b/c i notice the car feeling sluggish and then i pull the valve and put the number tdc and look at the flywheel the cams where 3-4 teeth off. That makes me wonder how can this be. Ive never had this problem before but after the h23vtec i've done about 3months ago i've been having this problem. Is it the belt and has anyone ever experienced something like this before.
