Clutch is slipping and not disengaging? FML!

so i drove to my friends house today, clutch felt fine, as we left i decided to get on it and go from 1st-2nd-3rd and all the times i went into a gear the clutch was slipping! also, every time i shifted i grinded a gear, so it leads me to think the clutch isn't disengaging all the way..
i've also installed a brand new Exedy stage 2 on my LS civic a few months ago, it has barely 10k miles..
my throwout bearing has been making noise for quite some time now, is it to the point where it's beat up and not disengaging and engaging the clutch as quickly as it should?
the question is, can my throwout bearing get so bad where if won't disengage the clutch as far as it should, and also not engage it quickly?
i have no visual site of any brake fluid leaking, the reservoir doesn't seem low.. but the fluid is very black and watery.. this would probably lead to the clutch not disengaging properly, but why would my clutch be slipping?
i'm stumped and my turn signals don't seem to be working! only my hazzards are.. grr

I have to agree.
Did you not replace the throwout bearing when you installed the "Exedy", if not, tisk tisk.
A bad throwout bearing will cause all sorts of grief, including the ones you have.
As mentioned you will have to pull the transmission, inspect for damage, replace the bearing.
I would also flush and replace the "clutch" fluid, inspect both the clutch master and slave cylinders. 94
PS, for the signal lights check the signal light fuse and the hazard switch.

before you pull out the tranny,
where is your freeplay? should be 3/4" of an inch +-
if it is adjusted too tight, it will not fully engage, and if it is way tight, along with not engaging, you may be over-stroking the pressureplate, pushing it into the clutch disc. that will cause it to not disengage.

I would also bleed the clutch system when you see black or watery brake fluid in your clutch reservoir. IMO, whenever I change my clutch or my friend's clutch, you should always change the release bearing and pilot bearing. I did on mine.

the throwout and pilot bearings were replaced (because they came with the clutch kit) the throwout didn't last long after it was installed though..
the freeplay feels the same as it's always been, about an inch or less..
but there is a signal light fuse for my blinkers?
