Why would she toss tranmission fluid like this?

Why would she toss tranmission fluid like this?
1994 Accord EX with 126k on it.
This is my girls car. It was low on transmission fluid (and slipping). I filled it up, seemed good for a bit, but I noticed the level was down to about the lowest tick mark in less than a month. I was going to fill it up again, but yesterday.
She starts the car, drives about 20 yards, says goodbye to me (in drive, foot on brake), and drives away. I see a puddle about 6 inches wide with bubbled fluid in it. Fluid has been clean/good in the past, and still is.
Thoughts?
My n00b thoughts
* axle seals
* transmission casing leak?
* pressure problem/overflow tube?
