alternator adjusting bolt

i just did the timing belt/water pump replacement on my moms 98 accord lx it has the f23a1 and i reassembled everything the way its supposed to be done. now everytime i drive it the alternator adjusting bolt keeps backing out releasing the tension on the alternator belt. any ideas on what i can do to fix this problem?
Did you install it correctly?
There's the bolt on the front underneath that has a little metal wing type thing, you slide the bolt through the break and alternator, then through that wing.
Loosely put on the alt's bolt then tighten down the adjustment bolt. Once it's tight crank down on the bottom bolt
