This is a major problem with the 6 Gen Accord's. I have every problem listed above plus crappy gas mileage.
Take your car to the dealership and let them see your vin number.

I had a slipping on a 99 Accord. Drained and refilled several times. Used Lucas additive the last drain. It helped for a few thousand miles, but eventually it returned.
