Rear bracket (7)
Driver's side bracket(5?)
Front trans mount(10)
A/C bracket(1)
You also need the drivers side engine mount if there isn't one, you can reuse your CX studs too.

where did you get a pic like that? that looks like its from a shop manual, is there a site that has a shop manual pica for hondas like that?

The only thing I needed was the rear mount out of the 99-00 Si. I used everything else off my DX....Maybe the AC bracket came on my LS motor. I cant remember. Good luck anyway
