93 honda civic dx crank pulley keeps come loose

Yea I have a 93 honda civic dx.tha crank pulley keeps *** loose and I tightin it twice but it keeps *** loose wat can it be tha bolt or sumthing else please help

did you check to make sure the keeper key is still i the pully?

check for the woodruff key thing. also are you torquing it down to over 140 ft lbs?
