Default clutch and clutch adjustment

Recently replaced a clutch on a stock 1998 Accord EX. 150K mi. ...Two issues remain ...

1.) The clutch now grabs just above the floor ...way too low. The play seems ok ...does not seem like i have any room to take any play out with adjusting the pedal. I've read on a forum somewhere that the clutch system compensates for wear. ... is there a way to re-set that auto adjustment ? How does that auto adjust work?
Any other ideas??
I bought the clutch at the local autoparts store ...should have been an OEM equivalent.

2.) I now have a squeak / rattle / whistle type sound as the clutch grabs. .... I did the stupid and unthinkable ...I did not have the flywheel turned ...by the time the flywheel was out, all the places were closed on that Sat and I needed the car monday ....Flywheel looked fine ...no grooves, no visible problems ....I sanded it well and kept it clean and re-installed ...I needed the car. ....
10K mi later, it is still doing it, but same or less than when it was first installed.

Anybody else run into a high pitched squeal / vibration from a new clutch? Sounds like the disk is probelmatic?
